Dr. Van Holtzman was a respectable and well-known asylum doctor and professional psychiatrist who was also the personal shrink of Noah Lewis and secretly, a psychopathic serial killer, who targeted women and would carve ritualistic symbols on his victims' cheeks.
He acted as a personal antagonist to Richard Castle as he was behind a traumatizing event which would haunt Castle for years and motivate him to become a writer.
During a final confrontation with Castle, Detective Kate Beckett was able to slip Castle her gun and he shot Van Holtzman to death. A news report later stated that the police began digging up his property in search of the bodies of his numerous victims.
